Role Overview & Responsibilities

This post is based at the Ealing satellite site Daycare and Outpatients within the Northwest Directorate. Moorfields is one of the world's leading eye hospitals, providing expertise in clinical care, research and education.

PROBATIONARY PERIOD

This post will be subject to a probationary period of 6 months.

During the probationary period, your suitability for continued employment will be assessed and provided a satisfactory standard is achieved and maintained, your employment will be confirmed. The probationary period may be extended

Or your employment terminated either during or at the end of probation, in line with the trust Probationary Policy. For the duration of the probation period, the notice period will be in line with contractual obligations on either side.

The post holder is responsible for assessment of care needs of patients, planning programmes of care, implementation and evaluation of these programmes with direct supervision.

At times they will have management responsibility for being in-charge of the ward and supervising junior members of staff. The post holder will be a key member in relation to the development of practice on the ward. They will be working independently/autonomously and at times without direct supervision, accountable for their own professional actions.

Ensuring the safety and well-being of service users, and always working in a manner that promotes dignity and human rights through the adoption of person centred care principles.

Clinical and Patient Care:
To support qualified staff in the assessment of health needs, planning, delivery and evaluation of care for a number of individuals or groups of patients, including clinical risk assessment.

To exercise a reasonable and agreed degree of professional autonomy and make judgements and decisions in order to satisfy the expectations and demands of the job.

To undertake clinical work and related tasks as delegated by qualified nursing staff following appropriate training and assessment of competency.

To liaise effectively with all stakeholders in the care process, including carers and relatives, aspiring to provide an effective, seamless and integrated service to patients.

To attain competencies, by following the Trusts required protocol. · To act as chaperone when required.

Professional Responsibilities:
To maintain and update relevant knowledge and skills, maintain a professional portfolio and participate in further training as appropriate, in order to enhance quality of care for patients and carers.

To undertake clinical supervision in line with Trust Policy, to further develop competence and clinical practice.

Communication:
To communicate effectively and in a timely manner within the team.

Understand the principles of confidentiality.

Always communicate in a way that maintains the patient’s confidentiality.

Education & Professional Development:
To develop within the framework of a personal development plan agreed with your manager.

To participate in training to achieve the competencies required to undertake the role under the supervision of a trained nurse.

Compliance and Governance

Maintain clear and legible patient records in accordance with both Trust and professional standards for record keeping.

In conjunction with the Manager and other senior members of the multi-disciplinary team support a positive culture and environment for both staff an patients.
