Role Overview & Responsibilities
We are looking for an experienced, dynamic practitioner passionate about homeless adults to join a year long pilot project aimed at helping improve access to health and social care for homeless people who attend the hospital Trust.
A dedicated team has been set up, featuring a specialist GP, dedicated homelessness nurse ,a housing specialist from charity Porchlight and a local council worker.
The practitioner will be supported and managed via the All Age Safeguarding team who provide support, advice, guidance and training to all staff across the Trust, based at QEQM hospital but expected to work Trust wide. A secondment from EKHUFT member of staff would be considered..
The pilot post holder will be responsible for the enhanced hospital management of homeless people referred to the team or identified by the team. The post holder will provide advice to medical & surgical teams, therapists & the Ward Sisters in discharging their patients in a safe and timely manner. The post holder will work closely with other members of the Homeless Team, specifically the Clinical Lead GP who will provide medical support to homeless people. The Homeless Team works closely with staff across all disciplines between hospital and community to ensure a safe, timely and effective transfer of patient care on discharge. The Homeless Team aims to improve the health of homeless people, provide holistic co-ordinated care, and reduce delayed discharges; and also reduce premature discharges of homeless patients with the aim of reducing future unscheduled admissions and ED attendances.
