Role Overview & Responsibilities

We are looking to recruit a compassionate and dynamic Advanced Clinical Practitioner to join the team at Appletree Medical Practice in Duffield.

This is a great opportunity for a diverse ACP, which includes working both with the frail & elderly population with the aim of keeping frail elderly well at home for longer and reducing hospital admissions, whilst also working as a primary care ACP, managing minor illness and common GP surgery presentations alongside the wider clinical team. Additionally, the post holder will proactively manage the health needs of care home residents through regular ward rounds and anticipatory care planning.

The practice prioritises education and development so is looking for a self-motivate, enthusiastic individual with a desire to learn.

A Band 8a position will be offered to candidates who have completed an MSc in Advanced Practice (or equivalent) and can demonstrate contemporary experience working at an advanced level. A Band 7 trainee position may be offered to an Aspiring ACP.

Our Surgery:
  • Purpose built surgery in Duffield, plus branch surgery at Little Eaton.

  • GP Teams comprises of: 6 GP partner practice with 2 salaried GPs, 2 GP trainees.
  • ACP Team comprises: 2 ACPs who support minor illness and long term conditions; 1 Trainee ACP employed by DCHS in the same role as this vacancy.

You will apply advanced clinical expertise, levels of judgement, discretion and decision making to the provision of clinical care to older people across primary and community care settings.

You will exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatment and management of patients, in particular the older person.

You will lead and monitor improvements to quality of care through clinical audit, evidence-based practice, clinical supervision, teaching and supporting professional colleagues and the provision of skilled professional leadership.

You will lead the implementation of relevant clinical guidelines into practice and contribute to the further development of the evidence base through research and audit.

If offered a trainee position you will commit to undertaking an apprenticeship programme comprising a 3-year MSc in Advanced Practice and the development of an e-Portfolio to demonstrate your capability across the four pillars.

You will be uplifted to Band 8a on successful completion of the MSc and successful submission of your portfolio to a cross-organisational review panel.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.
